3. Fill out the Military Appreciation Program Form with specific sailing information.
4. Fax the form to 305-436-4127 or email the form to militarydesk@ncl.com
   5. If you want to book under this, fill out Military Appreciation Form and mail it with deposit. Form has to be on file with NCL within 7 days of deposit. (The $500.00 per cabin deposit)
